Practical support for coordinating care

Ridgeline Care Network provides non-clinical support for neurodivergent people and their families who are managing health-care appointments, referrals, services, and information. We help you organize next steps, prepare questions, and communicate more clearly with the appropriate providers and agencies. We do not provide medical advice, replace your providers, or make clinical decisions.

What is care coordination?

Care coordination is practical support for organizing health-related next steps across providers, appointments, referrals, and information. For neurodivergent individuals and families, managing multiple doctors, diagnoses, and systems can feel overwhelming. Ridgeline helps you prepare for appointments, keep track of questions and follow-up items, understand information you receive, and communicate your needs more clearly. We do not replace your medical providers or make clinical decisions; we help you navigate the process with greater clarity and support.

Appointment support

We can help you prepare for appointments, organize questions, understand what to expect, and keep track of follow-up items. Ridgeline provides non-clinical support and does not provide medical advice, replace your provider, or make decisions about your care.

Benefits navigation

Benefits and insurance paperwork can be confusing. We can help you review notices, organize documents, identify questions, and prepare to contact the appropriate agency, insurer, provider, or benefits program. We cannot determine eligibility, guarantee coverage, or promise approval of an application or appeal.

Pastoral care

Our pastoral care offers optional spiritual and emotional support as a community ministry, at no charge. It is available to people of any faith tradition and to people with no faith tradition; participation is never required to receive other Ridgeline services. Pastoral care is not clinical mental-health treatment, crisis intervention, or a substitute for professional medical or behavioral-health care.

Facing challenges? We can help.

Care coordination may be helpful when health-care information, appointments, referrals, benefits, or follow-up tasks feel difficult to manage. Ridgeline offers practical, non-clinical support to help you organize information, prepare questions, and identify next steps with the appropriate providers, agencies, or programs.

Common challenges we address

You may be managing appointments with multiple providers, trying to understand referrals or follow-up instructions, preparing for a medical visit, or sorting through a notice from an insurer, Medicaid, or another program. Ridgeline can help you organize information, prepare for conversations, identify questions, and track practical next steps. We cannot interpret medical information, determine eligibility, or guarantee a particular decision, service, coverage, or outcome.

Our unique approach to care

Ridgeline Care Network provides independent, client-centered support that is focused on your goals and priorities. Our founder’s lived experience as a neurodivergent person navigating complex systems informs how we work: we use plain language, make expectations clear, and move at a pace that respects your needs. We offer sliding-scale fees to reduce financial barriers, subject to availability. Ridgeline provides non-clinical care coordination and does not replace medical providers, make clinical decisions, or guarantee outcomes from insurers, benefits programs, or other agencies.
